The Sequoia Hospital Foundation is honored to work with our donor community and our hospital colleagues to transform generous donations into essential support for patient care and programs. At their most recent meeting, the Foundation’s Board of Directors voted to provide funding for significant projects including:

  • Sequoia Community Care (SCC), a referral-based program, seeks to improve the health of “at risk” patients by ensuring a successful transition from hospital to home after discharge. The program enables older adults to age-in-place and avoid unnecessary re-hospitalizations for chronic conditions.
  • The 3rd Annual Heart and Vascular Symposium is a world-class forum convening national thought leaders in heart and vascular related fields for continued education, training, and discussion on emerging techniques being performed in centers of excellence across the country.
  • JointPoint is a navigation system for preoperative and intraoperative analysis and digital templating for orthopedic procedures. The system will help reduce surgical time and fluoroscopy exposure/use while improving surgical decision making that leads to better patient outcomes.
